To be eligible to ride on the school routes your child must have a completed School Route Application form on file at BCT before the child can ride. A new application form must be filed each school year. Click the link below for a copy of the ‘11-’12 school year application.
Title IIIB Application
Title IIIB is for people over the age of 60 who are living independently. Click the link below for a copy of the Title IIIB application. Once the form is completed, please send it to BCT at 328 Snedden Dr. Boone, IA 50036.
Low-income Assistance Application
BCT offers low-income transportation assistance for trips within Boone County. Assistance is provided on a prorated system based on household income, and may cover all or part of the trip cost for 10 trips per month. In order to be eligible for low-income assistance each person will need to complete and return the low-income assistance application. Special Trip Request
BCT offers special trips for groups who need transportation to events, such as group shopping trips or group outings. To request a special trip, fill out the Special Trip Request form below and fax the completed form to BCT at 515-432-6142. The request should be submitted at least one week prior to the date of the trip. For the complete list of group trip fares, see the Fares page of our website. All special trips are subject to driver availability and must meet the Federal Charter regulations. See the Fares page of our website for more information on the Federal Charter regulations.
